All seating requests will be processed and assigned randomly. Please follow these instructions carefully to ensure your party is seated together. • Please enter the first and last name of all the people you have purchase tickets for. Enter the first and last name, of the person who will be picking up the tickets for your group, as the first person on the list • Please complete this form even if your name will be the only one listed. If you have more than 8 people total in your party, please use a 2nd form. • Write each name as it appears on that Guest's Photo Id. Do not use a nickname or hyphenated name on the form unless you are sure that it will match their ID. • Be sure to indicate for each person, where indicated; Adult or Child (3-12); Dinner Choice (do not pick dinner choice for child guest); Any dietary restrictions; If you need any special assistance, please check the box and we will contact you. • Dietary Restrictions - Please indicate the specific dietary restriction for each person in the dietary restriction section of this form. THIS MEANS LIST EVERYTHING!! If you run out of room, please send us a separate email with the name of each person the and dietary restrictions list. • Special Assistance - Due to the venue seating, please make note of any Guests in need of special assistance, check the special assistance box of that guest listed on the form, so we may be sure to seat everyone appropriately. We will contact each Guest with differing abilities to discuss seating options prior to the event. This also includes baby/toddler assistance; if you need a highchair or any other special seating. • If you have friends who are attending that you did not purchase tickets for and you wish to sit next to them, please enter the first and last names of those people. We will seat you as close to them as possible. • Incomplete forms will be returned - Corrected forms (changes or additions) will not be accepted if received after the deadline (4/20/12).
